Andrew, thanks for working on this.
Unfortunately, 9.95 (installed, rebooted, verified version) still breaks when I go from listening to ripped MP3/Flac to Internet radio or vice versa. Squeezeslave instance disappears.
Stay listening to one type of "music" either local ripped or streaming internet, squeezeslave keeps on playing.
Again, thanks for the continued effort. Maybe SqueezeBoxServer 8.x will resolve the squeezeslave issue along with the syncing problems some people experience with 7.4x.
Hi yeswork - do you mind me asking how you are controlling the squeezebox that you are listening to?
By that I mean do you use the web interface for squeezebox server, control direct from your squeezebox (if so, what kind is it - boom, classic etc?) or use something like iPeng?
I found that when I used the web interface I had issues, but not so many when running softsqueeze (which I imagine is like using a real squeezebox?).
Also I've found if you hit 'pause' and then 'play' a few times that seems to get it working again - either ripped music (mp3's/flac) or internet radio/music.

I am a little confused by your question - but I will take a stab at answering. Please feel free to comment/add more questions.
1) I have a current installation of Vortexbox ISO on compatible hardware.
2) It runs great, except
3) After SqueezeBox Server upgrading to 7.4x, then SqueezeSlave the headless software music player, would mysteriously die of on its own.
4) SqueezeSlave only plays music on the local Vortexbox hardware/sound "card"
5) I typically use the Web interface to control the local squeezeslave and my "real" devices, which include a SB3 and Boom.
6) I occasionallly use IPeng to control any all devices.
As I see it, the problem is that 7.4x broke squeezeslave. Squeezeslave, being a competitor to buying real hardware from Logitech, is no longer supported by Logitech. The main authors of squeezeslave, who used to be independent, have since been hired by Logitech.
Squeezeslave 0.9.95 is still broken.
Fixing squeezeslave is a low priority for those individuals with the most knowledge.
End Users like me are not so happy.

Sorry, I forgot that when talking about Squeezeslave you meant playback from the Vortexbox itself.
Have you tried pausing playback, clearing the playlist (in the web interface, not sure about iPeng) and then changing to flac,mp3/internet radio? I think I found this helped when I was using my Vortexbox as a player.
I don't really use it much, so it's not really an issue for me, but I can see how it would be very frustrating. Here's hoping it does get fixed!
